Arthroscopic removal of apical and abaxial sesamoid fracture fragments in five horses.
Abstract: Five horses with apical and abaxial fractures of the proximal sesamoid bones were treated arthroscopically. Ipsilateral and contralateral triangulation were used to remove abaxial fragments; apical fragments were removed by ipsilateral triangulation. Excellent visibility of the fracture fragments was obtained with each method. Dissection and removal of fragments was accomplished with standard arthroscopic instruments, tendon-splitting knives, and curved scissors. Excellent functional and cosmetic results were achieved in all cases.

The study involves a veterinary procedure where five horses suffering from specific fractures in their proximal sesamoid bones were successfully treated using arthroscopic surgery. The authors reported that the visibility during the procedure was excellent and all treated horses had both great functional and cosmetic outcomes.
Research Context and Procedures
- In this research, the subjects were five horses diagnosed with apical and abaxial fractures in their proximal sesamoid bones. The proximal sesamoid bones are small bones located in the lower limbs of the horses. These fractures can cause major lameness in horses making treatment necessary.
- The researchers opted for arthroscopy, a minimally invasive surgical procedure, to treat these fractures. Arthroscopy involves the use of a small camera (arthroscope) inserted into the joint through a small incision to diagnose or treat joint problems.
Arthroscopic Removal Techniques
- The authors utilized ipsilateral and contralateral triangulation to remove the abaxial fracture fragments. In medical terms, ipsilateral refers to the same side of the body, whereas contralateral refers to the opposing side. Here, triangulation refers to the three-point approach used to access, visualize, and treat the fracture site.
- They employed ipsilateral triangulation only to remove the apical fracture fragments. The term apical refers to the tip or apex of a structure.
- Common arthroscopic instruments, like tendon-splitting knives and curved scissors, were used to dissect and remove the fractured bone fragments.
Results and Implications
- According to the study, excellent visibility was obtained during the arthroscopic procedures which is crucial in minimally invasive surgeries for proper diagnosis and treatment.
- The researchers reported excellent functional and cosmetic results in all cases post-surgery. This implies that the horses were able to regain normal function of their limbs and the cosmetic appearance of the surgical area was preserved or restored adequately.
- These positive outcomes suggest that arthroscopy could be a viable technique for managing apical and abaxial fractures of the proximal sesamoid bones in horses, presenting excellent recovery and minimal invasion.
