‘First Case of Stiff-Horse Syndrome in United Kingdom’.

Abstract: Stiff-person syndrome is a well-recognised disease in humans, whilst cases of stiff-horse syndrome (SHS) have been rarely reported in the veterinary medicine literature. SHS is a progressive immunomediated disorder of the central nervous system leading to a disturbance of gamma amino butyric acid transmission at the neuromuscular junction in which antibodies are produced against the enzyme glutamic acid decarboxylase. A 6 year-old Irish Sport Horse was presented for investigation of involuntary violent contraction of hindlimb and back muscles triggered by stress. The diagnosis of SHS was made after elevated anti-glutamic acid decarboxylase antibodies titres were detected in the horse's serum. Administration of oral corticosteroids (prednisolone 0.75 mg/kg bwt) resulted in resolution of the clinical signs. However, reoccurrence of clinical signs was observed after the dose of corticosteroids was reduced or the treatment discontinued. To the best of the authors' knowledge this is the first reported case of SHS in United Kingdom. Considering the potential implications on the horses' welfare, further investigation on SHS pathogenesis and treatment are warranted.

This research article reports the first known case of a rare neuromuscular disorder, stiff-horse syndrome (SHS), in the United Kingdom, in a 6 year-old Irish Sport Horse. The article further discusses the diagnosis, treatment, and the need for additional research.

Introduction

  • The research primarily deals with a rare neuromuscular disorder observed in horses known as stiff-horse syndrome (SHS).
  • SHS is characterized as a progressive immunomediated disorder that affects the central nervous system of the horse, causing disruptions in gamma amino butyric acid transmission at the neuromuscular junction.
  • In this disorder, antibodies are produced against the enzyme glutamic acid decarboxylase.

Case Presentation

  • The case involves a 6-year-old Irish Sport Horse, which began exhibiting violent contractions in its hindlimb and back muscles under stressful situations.
  • The horse was presented for examination due to these abnormal symptoms.

Diagnosis

  • Diagnosis of SHS was made on observing elevated levels of anti-glutamic acid decarboxylase antibodies in the horse’s serum.
  • This confirmation marked the first reported case of SHS in the United Kingdom, adding a significant milestone in veterinary medicine literature.

Treatment and Follow-up

  • After diagnosis, the horse was treated with oral corticosteroids (prednisolone) and showed resolution of the clinical signs.
  • However, when the dose of corticosteroids was reduced or the treatment was discontinued, symptoms reappeared.

Implications and Future Directions

  • This research draws attention to the need for further study in understanding SHS’s pathogenesis and treatment due to its potential impact on horse’s welfare.
  • Further investigations are necessary to develop a more effective treatment methodology and prevent recurrence once the horse has been taken off medication.
