[Idiopathic pericardial effusion with tamponade in a Friesian gelding].

Abstract: A 7-year-old Friesian gelding was referred to the Department of Large Animal Medicine and Nutrition of the Faculty of Veterinary Medicine because of an inadequate response to treatment for vague symptoms of colic. An extensive physical examination showed there to be circulatory problems, with right-sided decompensation. Heart sounds were muffled on both sides of the thorax, and ECG showed a low voltage and variable amplitude of the QRS complex. Further investigations indicated idiopathic pericardial effusion. Pericardiocentesis was performed and supportive therapy started. Three weeks later the circulatory system no longer showed abnormalities and the patient was discharged. When a patient presents with vague 'colicky' symptoms that cannot be explained by abnormalities of the digestive tract, the differential diagnosis should include the possibility of 'false' colic, of which pericardial effusion is one possible cause. Early diagnosis and treatment can contribute to a good result.

The research describes the occurrence of a rare heart condition known as idiopathic pericardial effusion in a 7-year-old Friesian gelding (horse). Discovered due to vague colic symptoms, the gelding was treated through pericardiocentesis and supportive therapy, eventually showing no further circulatory abnormalities.

Research Context and Objective

  • The article revolves around a clinical case of a 7-year-old Friesian gelding which was brought to the Department of Large Animal Medicine and Nutrition of the Faculty of Veterinary Medicine due to a perceived inadequate response to treatment for persistent but vague colic symptoms.
  • Upon detailed examination, the horse was observed to have circulatory issues, particularly right-sided decompensation, a condition where the heart cannot pump blood effectively.
  • The objective of the study was to diagnose and treat the unusual medical case effectively, which leads to the gelding’s recovery.

Findings and Diagnosis

  • A thorough physical examination showed that the gelding was suffering from more than just colic and there were signs of circulatory complications. Heart sounds were found to be muffled on both sides of the thorax, and Electrocardiogram (ECG) displayed a low voltage and variable amplitude of the QRS complex.
  • These indicators prompted further investigation, which led to the diagnosis of idiopathic pericardial effusion, a condition characterized by an abnormal accumulation of fluid around the heart. The term ‘idiopathic’ refers to the fact that the cause of the condition was unknown.

Treatment and Recovery

  • The researchers then carried out a pericardiocentesis, a procedure that involves removing the excess fluid from around the heart, and began supportive therapy to help the horse recover.
  • Three weeks after the treatment, the horse’s circulatory system was back to normal, showing no signs of abnormalities, and the patient was discharged.

Implications of the Research

  • The study suggests that when a patient presents with vague symptoms akin to colic which can’t be attributed to the digestive tract, practitioners should consider the possibility of a ‘false’ colic as a differential diagnosis. Idiopathic pericardial effusion could be a potential cause of these symptoms.
  • Timely diagnosis and appropriate treatment can significantly improve the prognosis in such cases, contributing to a good outcome.
