Ophthalmic cyclosporine in equine keratitis and keratouveitis: 11 cases.
Abstract: Topical cyclosporine A was safely used in a series of 11 cases of equine keratitis and keratouveitis and appeared to be an effective anti-inflammatory agent in 9 cases. The clinical diagnoses included interstitial keratouveitis, endotheliitis, multifocal punctate keratopathy and a melting stromal ulcer. In most cases, the presence or absence of insidious bacterial infection was not conclusively determined. Topical cyclosporine A had no deleterious effects in this series of cases. The authors suggest that topical cyclosporine in both aqueous and lipid base vehicles should be investigated and evaluated as an alternative mode of achieving ocular immunosuppression.

This research investigates the use of topical cyclosporine A for treating various eye diseases in horses and concludes it to be mostly effective and safe, encouraging further exploration into its use as an ocular immunosuppressant.
Objective and Methodology
- The researchers intended to evaluate the safety, effectiveness, and potential use of topical cyclosporine A in treating ocular conditions in horses.
- This drug was administered in 11 equine cases presenting with keratitis and keratouveitis, which are inflammation conditions of the eye.
- The specific diagnoses among these cases included interstitial keratouveitis, endotheliitis, multifocal punctate keratopathy, and a melting stromal ulcer.
Findings and Implications
- Out of the 11 cases, topical cyclosporine A appeared to be effective as an anti-inflammatory agent in 9 cases, demonstrating its potential use in the treatment of equine ocular inflammation.
- However, in most cases, it remained uncertain whether an insidious bacterial infection was present or absent. This aspect is crucial as it may influence the overall response to the drug.
- Importantly, throughout the study, it was found that cyclosporine A had no harmful effects on the animals, thus indicating its safe usage in horses.
- Given these results, the authors suggest that cyclosporine’s utility in both aqueous and lipid-based formulations should be explored in the future. This is to assess if it can serve as an alternative technique for bringing about ocular immunosuppression in equines.
- Ocular immunosuppression could be beneficial in managing and treating inflammatory or autoimmune conditions affecting the eyes of horses.
Conclusions
- Overall, the study supports the use of topical cyclosporine A in equine ocular treatment, proving its safety and efficacy in most cases.
- Further research is needed to assess its use in different formulations and to conclusively ascertain its role in managing bacterial infections commonly associated with ocular conditions.
