Pleuritis and pleural effusion in the horse: a study of 37 cases.

Abstract: Pleural effusion in 37 horses, including 15 acutely affected and 22 chronically affected, was found to be due to a variety of causes, including lymphocarcoma, pulmonary granulomas, coccidioidomycosis, equine infectious anemia, pulmonary abscesses, chronic pneumonia, and primary septic pleural effusion. Age, breed, or sex predilection was not found. Horses with chronic disease had weight loss, increased respiratory rate, dull respiratory sounds in the ventral portion of the thorax, and varying degrees of anorexia. Many horses were anemic. Those acutely affected had respiratory distress or signs of colic and many were anorectic. Most horses with acute primary disease had small volumes of pleural fluid. Culture and cytologic examination of pleural fluid and tracheal washings revealed the causative organism in some instances, but in a number of "primary" cases there were negative results on bacterial culture. The latter cases must be differentiated from other causes of chronic weight loss in the horse.

This research revolves around the study of pleural effusion in horses, identifying various causes and symptoms from a study of 37 affected horses, comparing between acute and chronic instances of the condition.

Study Context and Sample

  • The study examines pleural effusion in horses, a condition where fluid builds up in the pleural space around the lungs, rarely seen in horses and normally due to other underlying conditions.
  • 37 horses were examined for the study, 15 of which were acutely affected while the remaining 22 had chronic symptoms. Age, breed, or sex were not found to have any bearing on the condition.

Causes of Pleural Effusion in Horses

  • The causes identified for pleural effusion varied broadly and include lymphocarcoma (a type of cancer), pulmonary granulomas (small lumps of cells due to inflammation), coccidioidomycosis (a fungal disease), equine infectious anemia (a viral disease), pulmonary abscesses (the formation of pus-filled lesions), chronic pneumonia, and lastly primary septic pleural effusion (an infection in the pleural space).

Symptoms and Diagnosis

  • Chronic cases showed symptoms such as weight loss, increased respiratory rate, dull respiratory sounds from the lower part of the chest, anorexia of varying degrees, and anemia.
  • Acute cases showed symptoms like respiratory distress and signs of colic (abdominal pain) and many were also found to be anorectic.
  • Horses with acute primary disease had smaller volumes of pleural fluid.
  • Culture and cytologic examination of pleural fluid and tracheal washings were used to find the causative organism. However, the findings were inconclusive in a number of ‘primary’ cases which reported negative results on bacterial culture, showcasing a need for a more detailed diagnosis.

Implications of the Findings

  • The inconclusive diagnosis of some primary cases denotes a gray area that must be further explored. These instances must be differentiated from other causes of chronic weight loss in horses, hence a more thorough and conclusive diagnostic test is needed. It also highlights the complexity and diversity of causes leading to pleural effusion in horses.
