Recurrent Actinobacillus peritonitis in an otherwise healthy thoroughbred horse.
Abstract: A Thoroughbred gelding in North America was evaluated for Actinobacillus peritonitis on three different occasions over a 4-year period. At each presentation, peritoneal fluid had an elevated nucleated cell count (220,000-550,000 cells/µL) characterised by non-degenerate neutrophils, no visible bacteria, an elevated total protein (4.6-5.5 g/dL) and bacterial culture yielding Actinobacillus spp. Actinobacillus peritonitis appears to be a regional disease occurring in Australia and less commonly in New Zealand and North America. Recurrence, other than incomplete resolution, has not been previously reported. This case highlights the classical presentation, response to therapy and excellent prognosis despite the alarmingly abnormal peritoneal fluid characteristic of Actinobacillus peritonitis and questions the role of parasite migration in the pathogenesis. Finally, this case is remarkable because Actinobacillus peritonitis was recurrent over several years in an otherwise normal horse.

This research focuses on an unusual case of a healthy Thoroughbred horse that was diagnosed with Actinobacillus peritonitis, a bacterial infection, three times in a four-year span. It also questions the involvement of parasite migration in the cause of the disease and examines the horse’s recovery despite alarming symptoms.
Background and Case Presentation
- The study tracks the case of a healthy Thoroughbred gelding (castrated male horse) in North America who was found to have Actinobacillus peritonitis three times within a span of four years.
- This disease is principally a regional affliction, more common in Australia and to a lesser degree in New Zealand and North America. Multiple occurrences of the disease, barring incomplete resolution, have not been posted before.
- On every occasion, the horse’s peritoneal fluid showed increased cell counts, no visible bacteria, and an increased total protein. The bacterial culture consistently yielded Actinobacillus species.
Key Conclusions
- The case highlights the typical symptoms, response to treatment, and the promising prognosis despite intensely abnormal peritoneal fluid characteristic of Actinobacillus peritonitis.
- One of the critical points made through this case it that, despite the grim symptoms, the prognosis of Actinobacillus peritonitis can be quite positive.
- Given that the horse’s recurrence of Actinobacillus peritonitis over several years even though it is otherwise healthy is uncommon, this case study is quite unusual and important.
Suggestions for Further Research
- The paper suggests further exploration into the possible role of parasite migration in developing Actinobacillus peritonitis. This could shed more light on the pathogenesis – the biological mechanism that leads to the disease state.
- Further research might also identify potential strategies for preventing recurrent manifestation of the infection in other horses.
