Treatment of an Esophageal Stricture in a 1-Month-Old Miniature Shetland Colt.

Abstract: A 1-month-old Miniature Shetland colt was presented to the Burg Müggenhausen Equine Clinic. Primary complaints were regurgitation of milk, bilateral white nasal discharge, and weakness. Physical examination, endoscopy of the esophagus, and contrast radiography led to the diagnosis of an esophageal stricture and aspiration pneumonia. Surgical treatment by means of an esophagomyotomy was performed. The foal improved only temporarily and multiple sessions of endoscopic balloon dilation of the esophagus were performed afterwards. 12 months after the final treatment the foal was healthy and had no complaints regarding esophageal function.

This study details a successful surgical and endoscopic treatment conducted on a one-month-old Miniature Shetland colt that was suffering from esophageal stricture, resulting in numerous health complications including aspiration pneumonia.

Presentation and Diagnosis

  • A one-month-old Miniature Shetland colt was taken to the Burg Müggenhausen Equine Clinic with the primary complaints of milk regurgitation, white nasal discharge from both nostrils, and weakness.
  • Upon physical examination and further diagnostic procedures, which included an endoscopy and contrast radiography of the esophagus, the colt was diagnosed with an esophageal stricture and aspiration pneumonia. Esophageal stricture refers to the narrowing of the esophagus leading to difficulty in swallowing, while aspiration pneumonia is a lung infection that occurs due to the inhalation of foreign materials, in this case, regurgitated milk.

Surgical Treatment & Follow-up Interventions

  • Initially, the colt underwent surgical treatment using a procedure known as esophagomyotomy. This surgical procedure involves making cuts into the muscular layer of the esophagus to alleviate the esophageal stricture.
  • Despite the surgery, the recovery of the foal was only temporary. Consequently, multiple sessions of endoscopic balloon dilation of the esophagus were performed. Endoscopic balloon dilation is a nonsurgical procedure used to widen strictures in the esophagus by inflating a balloon at the area of narrowing.

Outcome & Prognosis

  • A year following the final treatment, the foal was reported to be healthy with no complaints related to esophageal function. This illustrates a successful long-term outcome for the combined surgical and endoscopic treatments applied on esophageal stricture.
