Ultrasonographic detection of chronic epididymitis in a stallion.
Abstract: Clinical signs of chronic epididymitis in a stallion included recurrent signs of inguinal pain--retraction of the right testis into the inguinal area, resentment of palpation of the right testis, and right-sided scrotal swelling. The tail of the right epididymis was firm and seemed to be adhered to the testis. The tail of the left epididymis felt firmer than normal. Ultrasonographically, the diameter of the head of the epididymis was considered larger than normal, was hyperechoic in relation to the right testis, and had an irregular border. Several bright 1- to 5-mm-diameter echogenic areas that alternated with less echogenic areas were seen in the head of the right epididymis. Ultrasonographic findings were interpreted as fibrosis attributable to chronic inflammation. Unilateral castration (right-sided) was performed, but the prognosis was guarded because of presumed involvement of the left epididymis.

The research focuses on the use of ultrasonography to detect chronic inflammation of the epididymis (a tube that carries sperm from the testicles) in a stallion, using clinical signs such as recurrent pain, abnormal firmness, and swelling. Unilateral castration was performed due to the severity of the condition, yet the prognosis remained uncertain due to potential involvement of the left epididymis.
Background
- In the study, a stallion exhibiting signs of chronic inflammation in the epididymis—the tube responsible for carrying sperm from the testicles—served as the subject. These clinical signs included recurring inguinal pain, abnormal firmness of the epididymis, and noticeable swelling in the scrotal region, among other symptoms. The specific symptoms included the stallion retracting its right testis into the inguinal region, sensitivity to touch (palpation) of the right testis, and swelling in the right side of the scrotum.
Diagnostic Methods
- The primary diagnostic method applied in this research was ultrasonography—an imaging technique that uses high-frequency sound waves to produce images of structures within the body. It was used to evaluate the condition of the epididymis.
- The diameter of the head of the epididymis was found to be larger than usual, and its borders irregular. It also appeared brighter (hyperechoic) in relation to the right testis.
- Within the head of the right epididymis, several bright areas—measuring between 1 to 5 millimeters in diameter—were detected. These brightly lit areas alternated with less echogenic regions.
Findings and Intervention
- The ultrasonographic findings were interpreted as evidence of chronic inflammation in the epididymis, resulting in fibrosis—a thickening and scarring of connective tissue caused by prolonged irritation, inflammation, or other forms of damage.
- Given the severity of the condition, unilateral castration (right-sided) was performed as an interventional measure on the subject. Despite this, the stallion’s prognosis was considered guarded, implicating potential involvement of the left epididymis in the observed condition.
