A protracted case of cryptococcal nasal granuloma in a stallion.
Abstract: A 13-year-old Standardbred stallion presented with a unilateral mucopurulent nasal discharge and airway obstruction was found to have an extensive cryptococcal nasal granuloma. Treatment was not attempted. The stallion was destroyed 26 months later having completed 2 successful breeding seasons. The granuloma had gradually increased in size and almost completely occupied the left nasal cavity and paranasal sinuses. It was characterised histologically by fibrous and myxomatous tissue and masses of yeasts. A cryptococcal granuloma found within the wall of the jejunum could represent evidence of dissemination, and organisms were seen within alveoli, bronchioles and the intestinal lumen.

The research article discusses a case study of a 13-year-old stallion diagnosed with a severe cryptococcal nasal granuloma, a fungal growth in the nasal passage, that was not treated and led to the horse’s euthanization over two years later, following evident progressive growth of the granuloma and possible dissemination to other parts of the body.
Case Presentation
- The research revolves around a case of a 13-year-old Standardbred stallion, which presented with symptoms including one-sided (unilateral) nasal discharge and airway obstruction.
- The horse was found to have a significant cryptococcal nasal granuloma, which is a fungal infection causing growths in the nasal cavity. Notably, no treatment was attempted in this case.
- Despite grappling with this condition, the stallion had two successful breeding seasons before it was euthanized 26 months after the initial diagnosis.
Progress and Evaluation of the Condition
- The granuloma or growth had gradually increased in size over time and had almost wholly filled the left nasal cavity along with the paranasal sinuses by the time of the horse’s death.
- Post-mortem histological examination of the granuloma revealed it to be composed of fibrous and myxomatous (mucous-like) tissue, with masses of yeasts, denoting the nature and composition of the fungal infection.
Possibility of Disease Dissemination
- A cryptococcal granuloma was also found within the wall of the horse’s jejunum (a part of the small intestine), suggesting the potential for the disease to spread beyond the nasal cavity.
- Furthermore, the same fungus was seen within the alveoli (air sacs in the lungs), the bronchioles (small air passages in the lungs), and the intestinal lumen (the interior space of the intestine), reinforcing the possibility of the condition’s dissemination to other body parts.
Concluding Remarks
- This case shows the progression of untreated cryptococcal nasal granuloma over an extended period in a horse and the potential for it to disseminate to other organs. It underscores the necessity for early detection and treatment to prevent severe complications or fatal outcomes.
