Arthroscopic debridement of subchondral bone cysts in the distal phalanx of 11 horses (1994-2000).

The study illustrates the successful use of arthroscopic debridement for the treatment of subchondral cystic lesions in the distal phalanx of young horses, which thereby contributes to their return to athletic activity.

Introduction

• The study challenges historical belief that conservative treatment of subchondral cystic lesions of the distal phalanx in horses generally results in a poor prognosis. It presents a new proposal in favor of surgical management of these lesions.

Objective

• The aim of the research was to evaluate the implications and results of arthroscopically-guided curettage on horses’ distal phalangeal subchondral cystic lesions (SCLs). This was done by examining the cases of 11 horses aged between 16 and 33 months that had undergone this treatment.

Methodology

• Medical histories of horses with lameness resulting from SCLs of the third phalanx that had been previously treated were carefully reviewed.
• The surgical procedure involving the arthroscopic debridement of SCLs was documented and detailed.
• Follow-up on the horses’ condition and athletic performance post-surgery was done through race records and direct contact with the horses’ owners and trainers.
• A statistical sign-rank test was applied to compare performance of the operated horses to that of their unoperated siblings.

Results

• The data showed that 91% (10 out of 11) of the horses studied were able to return to athletic soundness after the surgical treatment.
• The performance records of these horses post-surgery were similar to their siblings who had not undergone the operation, indicating no negative effect of the surgery on performance.

Conclusions

• The results suggest that the use of arthroscopic debridement for distal phalangeal SCLs is a promising treatment option for horses aged between 16 and 33 months, potentially leading to the horse’s successful return to its intended athletic activity.
• The study, however, limited the age group of the treated horses and didn’t include horses younger than 16 months or older than 33 months, which indicates that the outcome might change if the horses outside this group underwent the procedure.
• Documenting the treatment procedure and approach could raise awareness among the equine community, providing them with additional treatment options for this specific condition.
