Cardiac troponin I as compared to troponin T for the detection of myocardial damage in horses.

Abstract: Different cardiac troponin I (cTnI) assays give different results. Only 1 manufacturer has marketed troponin T (cTnT) assays. Therefore, cTnT often is preferred for detection of myocardial infarction in human patients. Studies of cTnT in horses are limited. Objective: To compare a cTnI and a high-sensitive cTnT assay (hs-cTnT) in horses. Methods: Cardiac troponin I and cTnT were determined in 35 healthy horses (group 1), 23 horses suspected to have primary myocardial damage (group 2a), and 41 horses with secondary myocardial damage caused by structural heart disease (group 2b). Methods: All cTnI samples were analyzed at laboratory A (limit of detection [LOD]: 0.03 ng/mL), whereas cTnT samples were analyzed at 2 laboratories with the same hs-cTnT assay (laboratory B, LOD: 10.0 pg/mL; laboratory C, LOD: 4.0 pg/mL). Results: The median cTnI concentration in group 2a (0.90 ng/mL; range, 0.03-58.27 ng/mL) was significantly higher (P < .001) than in group 1 (0.03 ng/mL; range, 0.03-0.09 ng/mL) or group 2b (0.05 ng/mL; range, 0.03-30.92 ng/mL), and the optimal cut-off for detection of primary myocardial damage was 0.095 ng/mL (sensitivity: 90.5%, specificity: 100%). Using an LOD of 10.0 pg/mL for all cTnT samples, a cut-off value of 10.5 pg/mL was found, but sensitivity was low (42.9%). When only samples analyzed at laboratory C (n = 58) were included, a cut-off of 6.6 pg/mL was found (sensitivity: 81%, specificity: 100%). Conclusions: Despite large quantitative differences, cTnI and cTnT are both useful for detection of myocardial damage in horses.

The paper explores the effectiveness of using cardiac Troponin I (cTnI) and high-sensitive cTnT assays in detecting myocardial damage in horses. It finds that while there are significant variations in the results, both cTnI and cTnT are useful for detecting this type of damage in horses.

Objective and Methods

The paper’s primary objective was to compare a cardiac troponin I (cTnI) and a high-sensitive troponin T (hs-cTnT) assay in horses. It was recognized that different manufacturers produced cTnI and only one produced cTnT. The study involved:

  • Determining cTnI and cTnT in 35 healthy horses (group 1), 23 horses suspected of having primary myocardial damage (group 2a), and 41 horses with secondary myocardial damage due to structural heart disease (group 2b).
  • Analysing all cTnI samples at one laboratory (lab A), while cTnT samples were examined at two different labs (lab B and lab C) using the same hs-cTnT assay.

Results

The results obtained in the study were:

  • The median cTnI concentration in group 2a was significantly higher than in group 1 or group 2b. However, both cTnI and cTnT proved useful in detecting myocardial damage in horses.
  • An optimal cTnI cut-off for detecting primary myocardial damage was 0.095 ng/mL with a high sensitivity (90.5%) and specificity (100%).
  • The use of cTnT with a limit of detection (LOD) of 10.0 pg/mL yielded a low sensitivity (42.9%), and an optimal cut-off value of 10.5 pg/mL was found.
  • When cTnT samples analysed exclusively at laboratory C were considered, a cut-off of 6.6 pg/mL was identified, with sensitivity of 81% and specificity of 100%.

The paper, therefore, concludes that despite significant quantitative differences in the results obtained using cTnI and cTnT, both are useful in detecting myocardial damage in horses.

Significance of the Study

This study is crucial for anyone involved in equine healthcare as it provides valuable information regarding the use of cTnI and cTnT as markers of myocardial damage in horses. While the sensitivity and specificity of these markers can vary depending on laboratory conditions, both have been shown to be effective tools in diagnosing myocardial injury, meaning they could play a key role in both the detection and subsequent treatment of cardiovascular diseases among horses.
