Combination of medical and surgical therapy for pleuropneumonia in a horse.

This research describes the treatment of a horse with an extrapulmonary abscess due to anaerobic pleuropneumonia. Traditional medical treatments were unsuccessful, leading to a surgical intervention known as intercostal thoracostomy, monitored by ultrasonography and video-endoscopy, and the horse was able to return to training after four months.

Treatment of Pleuropneumonia in a Horse

• The case centers on a 10-year-old Thoroughbred gelding (a castrated male horse) suffering from anaerobic pleuropneumonia, a severe respiratory illness primarily caused by anaerobic bacteria, which can lead to the formation of abscesses (pockets of pus) in the lungs.
• Traditional medical management treatments were ineffective in preventing the development of an extrapulmonary abscess. Extrapulmonary refers to outside the lungs, indicating that the abscess had expanded beyond the initial area of infection.

Intercostal Thoracostomy as Surgical Intervention

• Due to the failure of medical interventions, the researchers pursued a surgical procedure known as intercostal thoracostomy. This procedure involves creating an opening in the chest wall between the ribs to allow for the drainage of the abscess.
• The operation successfully achieved drainage, providing an effective solution to the abscess that had not responded to medical treatments.

Monitoring and Recovery

• Following the surgical intervention, the condition of the horse was closely monitored using ultrasonography (a diagnostic imaging technique based on the application of ultrasound) and video-endoscopy (a procedure where organs inside the body are viewed using a flexible tube with a light and camera at the end).
• The resolution of the abscess and subsequent bronchopleural fistulas (abnormal connections between the bronchi and pleural space) was closely observed through these techniques.
• Four months after the successful intervention and monitoring period, the horse was able to return to training, demonstrating a positive outcome in this case of combining medical and surgical treatments for pleuropneumonia in horses.
