Complications of Equine Musculoskeletal Surgery and Procedures.
Abstract: Complications of equine musculoskeletal procedures and surgery are an inevitable occurrence. This article summarizes complications principally encountered by primary equine veterinarians who perform and manage musculoskeletal procedures and surgeries. Topics discussed include complications of musculoskeletal diagnostic procedures, angular/flexural limb deformities, and muscle, tendon, and ligament surgery; surgery of the foot, orthopedic and arthroscopic surgery, and cast management. Each section lists complications encountered and includes diagnostic and treatment considerations. Awareness of these complications will empower equine veterinarians to identify and manage these adversities to result in the best possible outcomes for their patients.

Overview
- The article reviews common complications that arise during and after equine musculoskeletal surgeries and procedures.
- It is specifically targeted toward primary equine veterinarians performing these interventions to help them better manage and treat complications.
Detailed Explanation
Scope of Complications in Equine Musculoskeletal Procedures
- Complications are described as an unavoidable aspect of musculoskeletal surgeries and procedures in horses.
- The article covers a broad range of surgeries and procedures that involve the bones, joints, muscles, tendons, and ligaments of horses.
- Awareness of these complications is critical to enable timely identification and appropriate management, improving patient outcomes.
Types of Procedures Covered
- Musculoskeletal diagnostic procedures: These may include diagnostic imaging, arthrocentesis, or biopsies and can carry risks such as infection or tissue damage.
- Correction of angular and flexural limb deformities: Surgeries or interventions to address abnormal limb angulation or joint flexion, which can involve complications like incomplete correction, overcorrection, or growth disturbances.
- Muscle, tendon, and ligament surgeries: Procedures to repair or reconstruct soft tissue structures, with risks such as tendon rupture, adhesion formation, or delayed healing.
- Surgery of the foot: Including procedures on the hoof and distal limb, which may have complications like infection, laminitis, or poor wound healing.
- Orthopedic and arthroscopic surgeries: Involving bone and joint interventions, with complications potentially including joint infection (septic arthritis), implant failure, or cartilage damage.
- Cast management: Used postoperatively for immobilization but can lead to complications like pressure sores, swelling, or cast loosening.
Commonly Encountered Complications
- Infections: Resulting from surgical site contamination leading to cellulitis, abscess, or osteomyelitis.
- Delayed or poor wound healing: Due to vascular compromise, excessive tension, or contamination.
- Mechanical complications: Such as failure of implants, fracture displacement, or soft tissue damage.
- Neurological injury: From nerve entrapment or trauma during surgery.
- Overcorrection or undercorrection of deformities: Leading to limb dysfunction or the need for additional procedures.
- Adhesion and fibrosis: Causing restriction of joint or tendon movement and requiring intervention.
- Cast-related complications: Pressure necrosis, swelling causing compartment syndrome, or slippage of the cast.
Diagnostic and Treatment Considerations
- Early recognition of complications via clinical examination, imaging, and laboratory tests is essential.
- Treatment may include antibiotics for infections, surgical revision, wound care protocols, and supportive therapies like physical rehabilitation.
- Prevention strategies discussed involve meticulous surgical technique, appropriate perioperative care, and careful postoperative monitoring.
- Management plans should be individualized per patient and complication severity, involving referral when necessary.
Practical Value for Veterinarians
- The article serves as a guide for primary equine veterinarians, consolidating knowledge about potential surgical complications and their management.
- It empowers practitioners with information to improve early detection and outcome optimization.
- This resource supports decision-making around when to intervene and when to refer, contributing to better welfare and prognosis for equine patients undergoing musculoskeletal procedures.
