Complications of ophthalmic surgery in the horse.
Abstract: Corneal transplantation, amniotic membrane transplantation, phacoemulsification cataract extraction, and laser glaucoma therapy are routine ophthalmic surgical procedures in horses. This article discusses the indications, techniques, and postoperative complications of these and other ophthalmic surgical procedures in horses. Meticulous and accurate anatomic repair can minimize postoperative complications to maintain positive visual outcomes in ophthalmic surgery of the horse.

This research article focuses on the common ophthalmic surgical procedures in horses, such as corneal transplantation, amniotic membrane transplantation, phacoemulsification cataract extraction, and laser glaucoma therapy, and explains their indications, techniques, and potential postoperative complications.
Indications and Techniques of Ophthalmic Surgical Procedures
- Corneal transplantation is often performed on horses with corneal ulcers, perforations, or severe corneal diseases. The surgical technique involves removing the damaged part of the cornea and replacing it with a healthy corneal graft.
- Amniotic membrane transplantation is indicated in cases of severe corneal injuries, corneal ulcers, or when corneal healing is delayed. The procedure involves placing amniotic membrane over the corneal surface to promote healing and provide pain relief.
- Phacoemulsification cataract extraction is indicated in horses with cataracts causing significant vision impairment. The technique involves the use of ultrasound to break the cataract into small pieces, which are then removed.
- Laser glaucoma therapy is used in cases of glaucoma to reduce intraocular pressure. The laser is applied to the trabecular meshwork, increasing fluid outflow and thereby lowering intraocular pressure.
Postoperative Complications and Their Management
- Postoperative complications of corneal transplantation may include graft rejection, infection, or corneal melting. These can be managed with antibiotics, anti-inflammatory drugs, or repeat surgery if necessary.
- Amniotic membrane transplantation may result in complications like membrane dislocation or infection, which can be managed with sutures for proper attachment and antimicrobials, respectively.
- Complications following phacoemulsification cataract extraction may include inflammation, infection, or retinal detachment. These can be managed with anti-inflammatories, antibiotics, or retinal reattachment surgery.
- Laser glaucoma therapy may result in complications like a sudden decrease or increase in intraocular pressure. This would be typically managed with additional medications or repeat laser procedures.
The Importance of Accurate Anatomic Repair
The researchers emphasize that meticulous and accurate anatomic repair during surgery is vital to minimize postoperative complications and to maintain positive visual outcomes. This calls for a deeper understanding of the anatomy of the horse’s eye, precise surgical techniques, and careful postoperative management.
