Cranial approach to the humerus for repair of fractures in horses and cattle.
Abstract: A cranial approach to the humerus was developed for application of a compression plate for repair of comminuted humeral fractures in a weanling foal, a calf, a heifer, and a mature cow. An incision was made from the cranial portion of the greater tubercle of the humerus to the radius. The cranial aspect of the humerus was exposed by retraction of the brachiocephalicus muscle and blunt dissection between the biceps brachii and brachialis muscles. A second plate was applied laterally in the heifer and cow by retraction of the brachialis muscle and elevation of the extensor carpi radialis muscle. Successful fracture reduction, alignment, and stabilization were achieved in all four animals.

The article outlines a cranial approach for the repair of complex fractures in the humerus of horses and cattle using a compression plate. The technique was successfully applied to a young foal, calf, heifer, and mature cow.
Overview of the Procedure
- The cranial approach to the humerus or upper arm bone begins with an incision from the cranial portion of the humerus’ greater tubercle to the radius, i.e., the larger of the two bones in the forearm.
- The front part of the humerus was exposed by retracting the brachiocephalicus muscle, a large muscle of the upper limb, and performing blunt dissection between the biceps brachii and brachialis muscles. These are the major muscles of the upper arm that are responsible for bending and extending the elbow joint.
Application of the Second Plate
- In the case of the heifer and cow, a second plate was applied on the side by retracting the brachialis muscle and lifting the extensor carpi radialis muscle. This is one of the muscles that allow for extension and abduction at the wrist joint.
Results of the Procedure
- The procedure resulted in successful fracture reduction, alignment, and stabilization in all four animals. This indicates that the fractures were effectively minimized, the bones were correctly aligned, and the stabilization of the bones was achieved, ensuring the most optimal healing scenario.
Implications of the Study
- This study presents a new approach for treating complicated humeral fractures in these large animals, which can significantly impact their quality of life and productivity. If successfully replicated, this procedure has wide-ranging implications for veterinary medicine and the treatment of similar types of fractures in different kinds of large animals.
