Effectiveness of a paraverterbral nerve block versus local portal blocks for laparoscopic closure of the nephrosplenic space: A pilot study.

Summary
The research compared the effectiveness and implications of using a paravertebral nerve block versus local portal blocks for laparoscopic closure of the nephrosplenic space in horses. The study concluded that both methods provided similar conditions during surgery and can be used interchangeably for this procedure.
Methodology and Procedures
The study was conducted as a randomized clinical trial involving twelve horses. These were divided randomly into the two study groups:
- Group one received a paravertebral block of nerve T , L , and L.
- Group two received local blocks of the three laparoscopic portals.
During the procedure, horses were sedated using dexmedetomidine and morphine. Depending on the group they were in, each horse was injected with either lidocaine or saline into each paravertebral nerve site or into each local portal site to aid in the laparoscopic closure of the nephrosplenic space.
Assessment and Scoring
The overall quality of sedation, analgesia, behavior and ease to carry out the surgery were evaluated and scored with the use of a visual analog scale (VAS).
Results and Findings
The study found that the time needed to complete local anesthesia was similar for both groups, indicating comparability in efficiency for both types of blocks. It also found that the VAS scores between the two groups didn’t differ, suggesting a similar level of efficacy in terms of sedation, pain relief, and behavior management. Furthermore, the total duration of surgery was similar between the two groups.
Conclusion and Implications
The results indicate that the paravertebral nerve block and the local portal blocks offer comparable conditions during laparoscopic surgery for the closure of the nephrosplenic space. Therefore, both methods can be used interchangeably in surgical procedures. This flexibility can be beneficial in various surgical situations depending on individual circumstances, enhancing the versatility of treatment options for veterinarians.
