Equine recurrent airway obstruction: pathogenesis, diagnosis, and patient management.
Abstract: Recurrent airway obstruction is a condition that affects some older horses maintained in confinement. Clinical signs range from exercise intolerance with occasional cough to dyspnea at rest. Bronchoalveolar lavage cytology is characterized by neutrophilic leukocytosis (15%-85%) and is recommended for making the diagnosis in horses with mild to moderate disease. Environmental management combined with periodic bronchodilator and antiinflammatory corticosteroid therapy yields the best prognosis for disease remission.

The research article explores equine recurrent airway obstruction, its causes, how to diagnose it, and how to manage affected horses. It covers a range of symptoms and recommends a combination of environmental management, bronchodilator and corticosteroid therapy for effective treatment.
Understanding Recurrent Airway Obstruction in Horses
- Equine Recurrent Airway Obstruction (RAO) is a health condition often found in older horses who spend most of their time in confined spaces.
- This ailment can manifest a range of clinical signs – mild symptoms such as exercise intolerance and occasional coughing, to more severe ones like difficulty breathing (dyspnea) even when the horse is at rest.
Disease Diagnosis
- The research suggests the use of bronchoalveolar lavage cytology, a diagnostic procedure involving the microscopic examination of cells extracted from the lungs, to diagnose this condition accurately.
- Typically, the cytology results of horses suffering from RAO display neutrophilic leukocytosis, a condition characterized by an abnormally high number of neutrophils, a type of white blood cell. The percentage of neutrophils is usually in the range of 15%-85%.
- This method is especially recommended for cases where the affliction exhibits mild to moderate symptoms.
Managing Affected Horses
- A comprehensive management approach combining environmental management and medical therapy has been proposed for effective treatment.
- As a part of this approach, horses should be kept in a more open and spacious environment while being administered bronchodilators and anti-inflammatory corticosteroids periodically.
- Such methodology offers the best prognosis for disease remission, meaning it can significantly improve the horse’s health and possibly lead to complete alleviation of symptoms.
