Ileal diverticulum as a cause of chronic colic in a horse.
Abstract: A 4-year-old Thoroughbred gelding with a history of chronic colic was referred for evaluation of signs of abdominal pain. Exploratory laparotomy revealed a diverticulum of the distal portion of the ileum located between the layers of the mesentery. The diverticulum was similar in location and appearance to acquired ileal diverticula described in human beings. Complete bypass of the diverticulum by ileocecostomy resulted in apparent resolution of the chronic colic in this horse.

This research study focused on a chronic colic case in a Thoroughbred gelding horse that was resolved after finding and addressing an ileal diverticulum, a pouch formation in the intestines. The successful treatment applied was an ileocecostomy, bypassing the diverticulum.
Introduction and Background
- The study describes a case of chronic colic in a 4-year-old Thoroughbred gelding. Chronic colic is a term used to describe recurring episodes of abdominal pain in horses, often linked to problems in the digestive system.
- The horse in question was referred for evaluation due to ongoing signs of abdominal pain, which indicated a severe and recurring health issue.
Identification of the Problem
- An exploratory laparotomy, a surgical operation involving an incision into the abdomen, was conducted to locate the source of the horse’s abdominal pain. This technique is frequently used to identify underlying issues when non-invasive methods are unsuccessful.
- Through this surgical exploration, the researchers identified an ileal diverticulum. This is an uncommon condition in horses where a pocket or pouch develops in the wall of the ileum, a part of the small intestine. Ileal diverticulum can lead to complications such as inflammation or obstruction.
- The discovered diverticulum was located between the layers of the mesentery, part of the peritoneum that encircles the intestines and other organs in the abdomen.
- The study remarks upon the similarity of this case to acquired ileal diverticula in humans, providing a comparative clinical perspective.
Treatment and Outcome
- To address the issue, the team performed an ileocecostomy, a surgical procedure that creates a direct connection between the ileum and the cecum, the first part of the large intestine. This bypasses the diverticulum, allowing for normal passage of an intestinal content without the disturbance caused by the pouch.
- The horse experienced an apparent resolution of its chronic colic after the surgery, indicating that the procedure successfully addressed the root cause of the ailment.
- This successful outcome suggests that undiagnosed ileal diverticula may be an underlying cause in some unresolved chronic colic cases in horses. It also affirms the effectiveness of ileocecostomy for treating this specific condition.
