IMAGING DIAGNOSIS – UNILATERAL TRIGEMINAL NEURITIS MIMICKING PERIPHERAL NERVE SHEATH TUMOR IN A HORSE.
Abstract: A 16-year old Warmblood gelding presented with a nonhealing corneal ulcer and absent corneal sensation in the left eye. A lesion affecting the maxillary and ophthalmic branches of the left trigeminal nerve was suspected. Magnetic resonance (MR) imaging identified marked thickening of the ophthalmic and maxillary branches of the left trigeminal nerve. The nerve was iso- to hypointense on T1-weighted and T2-weighted images with heterogeneous enhancement. A peripheral nerve sheath tumor was suspected, however granulomatous neuritis was histopathologically confirmed. These inflammatory changes can result in severe nerve enlargement and should be considered with MR findings suggestive of peripheral nerve sheath tumor.

This article discusses a case where a 16-year-old horse presented symptoms that initially suggested a nerve tumor, but was later identified through imaging and pathological examination to be severe inflammation of a facial nerve, proving that inflammation can lead to similar diagnostic indicators as a tumor.
Introduction
- Imaging Diagnosis – Unilateral Trigeminal Neuritis Mimicking Peripheral Nerve Sheath Tumor in a Horse is a research article that elaborates a rare and important clinical case of a misdiagnosed peripheral nerve sheath tumor in a 16-year-old Warmblood gelding.
Preliminary Diagnosis
- The horse presented with symptoms of a nonhealing corneal ulcer and a total absence of corneal sensation in its left eye, which led the veterinarians to suspect that the issues were originating from a lesion in the maxillary and ophthalmic branches of the left trigeminal nerve.
Magnetic Resonance Imaging
- A magnetic resonance imaging scan was performed to get a more accurate look at the supposed lesion.
- The images identified marked thickening of the ophthalmic and maxillary branches of the left trigeminal nerve.
- These branches of the nerve appeared to be iso- to hypointense on T1-weighted and T2-weighted images. They also showed a heterogeneous enhancement of the nerve, which is usually seen in tumors.
- These signs led to an initial presumption of a peripheral nerve sheath tumor.
Final Diagnosis
- A histopathological examination was performed to confirm the diagnosis. It yielded unexpected results, revealing that the nerve was not affected by a tumor, but was instead experiencing granulomatous neuritis, a severe inflammation.
- This case illustrates the fact that an inflammatory condition, such as granulomatous neuritis, can mimic the imaging characteristics of a peripheral nerve sheath tumor. This is due to the fact that these inflammatory changes can cause severe nerve enlargement.
Implication
- This research points out that clinicians need to consider the possibility of severe inflammatory conditions when magnetic resonance imaging findings indicate the presence of a potential peripheral nerve sheath tumor. This can help prevent misdiagnosis and ensure the implementation of appropriate treatment.
