Immune-mediated hemolytic anemia in a horse.
Abstract: An 18-year-old Quarter Horse gelding was determined to have immune-mediated hemolytic anemia after detection of autoagglutination of RBC spherocytosis as well as a positive direct Coombs test result. A lack of response to treatment with corticosteroids necessiated the administration of cyclophosphamide and azathioprine. The anemia resolved after treatment with chemotherapeutic drugs.

This article describes the case of an older horse diagnosed with immune-mediated hemolytic anemia, a condition where the immune system attacks red blood cells. The horse was successfully treated with chemotherapeutic drugs after other treatments failed.
Diagnosis
- The research presents the case of an 18-year-old Quarter Horse gelding (a castrated male horse) who was diagnosed with immune-mediated hemolytic anemia. This condition occurs when the immune system wrongly identifies red blood cells as a threat and destroys them, leading to a state of anemia.
- The diagnosis was confirmed through the detection of autoagglutination and RBC spherocytosis. Autoagglutination refers to the clumping together of the red blood cells, while spherocytosis is the condition where the cells take up a spherical shape instead of the usual concave one. These are common signs of an immune response targeting red blood cells.
- A positive result from a direct Coombs test further confirmed the diagnosis. The Coombs test checks for the presence of certain antibodies that can bind to and destroy red blood cells.
Treatment
- Initially, the horse was treated with corticosteroids, which are commonly used to reduce inflammation and suppress immune response. However, in this case, the horse showed no improvement or response to this treatment.
- Due to the failure of corticosteroids, the horse was then treated with two chemotherapeutic drugs, cyclophosphamide and azathioprine. These medications are known to suppress the immune system and are often used in cases of severe immune response or autoimmune diseases.
- The horse’s anemia resolved after the administration of these chemotherapeutic drugs. This suggests that the aggressive approach of using chemotherapeutic drugs was effective in controlling the immune response and restoring the horse’s red blood cell count.
