Intermittent hypoglycemia in a horse with anaplastic carcinoma of the kidney.
Abstract: Clinically apparent hypoglycemia is rare in adult horses. Hypoglycemia is a well-recognized paraneoplastic syndrome in humans and dogs with non-insulin-secreting tumors and may occur in horses as well. Hypoglycemia associated with non-insulin-secreting tumors is believed to result from production of an abnormal form of insulin-like growth factor II. Neoplasia should be considered in the differential diagnosis for adult horses with hypoglycemia.

The research study investigates the occurrence of intermittent hypoglycemia in an adult horse suffering from anaplastic carcinoma of the kidney, suggesting that tumors may cause low blood sugar levels in horses by producing an abnormal insulin-like growth factor II.
Research objectives and context
- The aim of this research was to investigate the unusual case of a horse with clinically apparent hypoglycemia, a condition that is infrequently observed in adult horses.
- The phenomenon of hypoglycemia, or low blood sugar levels, is widely recognized in humans and dogs as a paraneoplastic syndrome, a condition that is the result of cancer but not directly related to the physical influence of the cancerous cells.
- The researchers hypothesized that a similar hypoglycemic paraneoplastic syndrome could potentially occur in horses as well.
Role of non-insulin-secreting tumors
- One proposed cause of hypoglycemia is the presence of non-insulin-secreting tumors in the body. These tumors do not secrete insulin but are believed to produce a variant of insulin-like growth factor II, which can interfere with the regulation of blood sugar levels.
Differential diagnosis for adult horses with hypoglycemia
- Given the findings in this study, the researchers suggest that the possibility of neoplasia (formation or presence of a new abnormal tissue or tumor) should be considered when diagnosing adult horses presenting with hypoglycemia.
Implications of the research
- This research has implications for equine health and veterinary practice, prompting a more thorough investigation into the potential causes of hypoglycemia in adult horses. It suggests that the presence of non-insulin-secreting tumors might be of particular interest.
- In addition, the recognition and diagnosis of paraneoplastic syndromes in horses could contribute to early detection and management of underlying malignant conditions like the anaplastic carcinoma of the kidney mentioned in the study.
