Primary distension of the guttural pouch lateral compartment secondary to empyema.
Abstract: A 6-year-old, 420-kg quarter horse gelding was presented with a 2-month history of difficulty swallowing and dyspnea. The horse was diagnosed with a right guttural pouch empyema with many large chondroids. Two surgeries were required to completely remove all the chondroids from what proved to be a primary distension of the guttural pouch lateral compartment.

The paper discusses an unusual case of a horse suffering from severe swallowing difficulties and breathlessness, which was subsequently diagnosed with a condition involving infection (“empyema”) and abnormal growths (“chondroids”) inside a pouch-like structure in its throat known as the guttural pouch. Surgical intervention was required to remedy the condition in this horse.
Case presentation
- The research describes the case of a 6-year-old Quarter Horse gelding that weighed 420 kg. The horse had been demonstrating symptoms such as trouble swallowing (dysphagia) and shortness of breath (dyspnea) for a period of two months.
Diagnosis
- Upon examination, the horse was found to have a guttural pouch empyema on the right side. The guttural pouch is a unique anatomical feature in horses that can sometimes develop infections, known as “empyema”. This specific case was further complicated by the presence of many large, unusual growths known as “chondroids”.
- The formation of chondroids is a rare but serious complication of guttural pouch empyema. Chondroids are hard, mass-like formations of pus that are often difficult to remove due to their size and location.
Treatment
- The horse underwent two surgical procedures to remove all the chondroids found in the guttural pouch. Such surgical intervention was essential because leaving chondroids untreated can lead to more serious complications, such as persistent infection, difficulty swallowing, and breathing problems.
- The issue faced by the horse was discovered to arise from a primary distension of the guttural pouch’s lateral compartment. Distension in this context refers to an abnormal enlargement or expansion of the pouch.
Study Significance
- The research signifies the importance of diagnosing and treating conditions like guttural pouch empyema swiftly and effectively to prevent further complications. It underscores the potential complication of chondroid formation and the requirement of surgical procedures to mitigate such issues.
- The paper also highlights the need for more research into understanding the pathogenesis and effective treatment of primary distension in the guttural pouch’s lateral compartment.
