Rib osteomyelitis in three foals.
Abstract: Three foals aged between 21 and 44 days were presented for evaluation of pyrexia and lameness. Each had subcutaneous abscessation associated with osteomyelitis of a rib at the costochondral junction and pathological fracture of the affected rib; one foal had more than one rib affected. Other localised sites of infection included other sites of osteomyelitis, omphalophlebitis, uveitis, enterocolitis, and calcaneal bursitis. The subcutaneous abscessation and rib osteomyelitis was treated surgically in two foals. Salmonella typhimurium was isolated from the subcutaneous abscess in one foal and from the faeces of another. Two of the three foals were euthanased. Ultrasonography was a more useful modality than radiography for establishing the diagnosis of costal osteomyelitis.

This study focuses on three foals (young horses) who were experiencing fever and lameness, ultimately diagnosed with an unusual infection in their ribs. The research found that ultrasonography was more effective than traditional radiography in diagnosing this condition, and aggressive treatment was necessary but not always life-saving.
Overview of Cases
- The research involved three young foals aged between 21 and 44 days. They were initially brought in due to symptoms of fever and lameness.
- The examination found each of them to have subcutaneous abscessation, a condition in which pus-filled cavities formed beneath the skin. This was linked to a condition known as osteomyelitis at the costochondral junction (where the ribs connect with the costal cartilage) which is a bone infection.
- A pathological fracture was also identified in the affected rib of each foal, and in one of these foals, more than one rib was affected by the condition.
- Apart from these primary symptoms, each foal showed signs of localized infections in different areas such as enterocolitis (an inflammation of the digestive tract), omphalophlebitis (an infection of the umbilical cord), uveitis (an inflammation of the middle layer of the eye), and calcaneal bursitis (an inflammation of the heel).
Diagnosis and Treatment Procedures
- The researchers isolated the bacteria Salmonella typhimurium from the subcutaneous abscess of one foal, and from the feces of another, indicating a contributing factor to the observed symptoms.
- The rib osteomyelitis and subcutaneous abscessation were treated surgically in two out of three foals. Despite comprehensive treatment efforts, two out of the three foals had to be euthanized due to the severity of the health conditions.
Techniques for Assessment
- Notably, the research found that ultrasonography was a more useful method than traditional radiography for diagnosing this rib condition known as costal osteomyelitis.
- Ultrasonography, an imaging method that uses sound waves, was better able to pick up on the signs of this infection within the rib cartilage, demonstrating its merit in such complex diagnoses.
