Transcervical endoscope-guided emptying of a transmural uterine cyst in a mare.
Abstract: An 18-year-old Friesian mare with a large intrauterine cyst was examined by transrectal ultrasonography. There were several small to moderately sized intraluminal endometrial cysts, one of which connected via the myometrium to a large subserosal cyst, thereby effectively forming a 'transmural' cyst complex. During a videohysteroscopy, the intraluminal part of this transmural cyst was removed by electrocoagulation via a polypectomy snare. It was then possible to drain the large subserosal part of the cyst into the uterine lumen by transrectal massage, thereby confirming the presence of the transmural connection.

This study examines the treatment of a large uterine cyst in a horse using transrectal ultrasonography and videohysteroscopy. The cyst was removed through electrocoagulation and then drained via transrectal massage confirming the connection between intrauterine and subserosal cysts.
Research Background and Objective
- The research revolves around a case study of an 18-year-old Friesian mare that was diagnosed with a large intrauterine cyst. The purpose of the study was to investigate the efficacy and procedure of treating such conditions using transrectal ultrasonography and videohysteroscopy.
Methodology and Procedure
- The mare was first given a thorough examination using transrectal ultrasonography. This identified several small to moderately sized endometrial cysts.
- One of these intraluminal endometrial cysts was found to be connected to a larger subserosal cyst via the myometrium, effectively forming a ‘transmural’ cyst complex.
- During a videohysteroscopy, the intrauterine part of this transmural cyst was removed through a method known as electrocoagulation. The researchers used a polypectomy snare for this procedure, which involves cutting and cauterizing tissues to prevent excessive bleeding.
Results and Findings
- After the electrocoagulation procedure, it was possible to drain the larger subserosal part of the cyst into the uterine lumen. This was done through a process known as transrectal massage.
- The successful drainage confirmed the presence of the transmural connection, i.e., the connection between the smaller intrauterine cysts and the larger subserosal cyst.
Conclusion
- The study concluded that transrectal ultrasonography and videohysteroscopy, coupled with electrocoagulation via a polypectomy snare, can be effective methods for the treatment of large intrauterine cysts in mares. This approach allows for the safe removal and drainage of such cysts.
