Treatment of coccidioidomycosis osteomyelitis with itraconazole in a horse. A brief report.

Abstract: Itraconazole, a tricyclic azole effective against a number of deep mycotic diseases, was used to treat a Quarter Horse filly with coccidioidomycosis. The horse was almost normal after 90 days of treatment. Five months after discontinuing itraconazole treatment, the filly had severe neck pain and neurologic signs from recurrence of coccidioidomycosis and was treated with itraconazole for an additional 6 months. Her clinical condition improved to almost normal and the filly has remained normal for 2 years. There was no evidence of drug toxicity.

This research article describes the successful treatment of a horse suffering from coccidioidomycosis osteomyelitis using a drug called itraconazole. After undergoing this treatment, the horse’s condition had considerably improved and it has been healthy for two years with no signs of drug toxicity.

Understanding Coccidioidomycosis and its Implication

  • Coccidioidomycosis, or “Valley Fever,” is a type of fungal infection often found in soil and dust in specific regions, with the possibility of affecting various mammals, including humans and horses.
  • In horses, it commonly affects the bones, leading to a condition known as osteomyelitis. Signs include severe pain, neurological symptoms, and other systemic complications.

Role of Itraconazole in the Treatment

  • Itraconazole is a tricyclic azole medication that’s known to be effective against a range of deep mycotic or fungal diseases, including coccidioidomycosis.
  • In this case study, it was used to treat a Quarter Horse filly affected by this condition. It’s noteworthy because it represents a novel use of the compound in veterinary applications.

Course of Treatment and Outcome

  • The horse was treated with itraconazole for 90 days initially, leading to a significant improvement in symptoms.
  • However, five months after this treatment was stopped, the horse showed signs of recurrence, presenting with severe neck pain and neurological symptoms.
  • The researchers then initiated a second, more extended round of treatment with the compound, which lasted for 6 months.
  • This round of the treatment led the horse’s condition to improve substantially, and it has remained stable and symptom-free for 2 years following the treatment.
  • Importantly, throughout this process, there were no signs of drug toxicity, indicating that the treatment was safe as well as effective.

Implications of the Study

  • This study, while based on a single case, suggests that itraconazole can be an effective treatment strategy for horses with coccidioidomycosis osteomyelitis.
  • Not only was the treatment effective, but it proved safe over both short- and long-term durations, with no signs of drug toxicity.
  • This could potentially lead to expanded use of the compound in veterinary applications, particularly in areas where coccidioidomycosis is prevalent.
