Horses and zebras: probabilities, uncertainty, and cognitive bias in clinical diagnosis. Medical diagnosis is typically an iterative process guided by integration and synthesis of data into a model of disease. However, facts are not the only inputs into this process. A case of medical mis-diagnosis is presented, in which systematic cognitive bias is considered to have played a role in generating error. Specific cognitive biases are cited, and measures that can be taken to minimize their negative impact are reviewed.
The effects of hormone replacement on the biomechanical properties of the uterosacral and round ligaments in the monkey model. The purpose of this study was to determine effects of ovariectomy (OVX) and conjugated equine estrogens plus medroxyprogesterone acetate (CEE/MPA), or ethinyl estradiol plus norethindrone acetate (EE/NA) on biomechanics of uterosacral (USL) and round (RL) ligaments in postmenopausal (PMP) monkeys. Methods: This was a randomized, triple blind, placebo-controlled study. OVX monkeys received 12 months no treatment (Pbo) (n = 19), CEE/MPA (n = 19), or EE/NA (n = 21). USL and RL step strains and stress-relaxation data were curve-fitted, giving strain-dependent tensile modulus (TM) from 0% to 30%. R...
Reduced vaginal bleeding in postmenopausal women who receive combined norethindrone acetate and low-dose ethinyl estradiol therapy versus combined conjugated equine estrogens and medroxyprogesterone acetate therapy. The purpose of this study was to compare the effects on vaginal bleeding patterns of continuous combined hormone replacement therapy with norethindrone acetate and ethinyl estradiol versus conjugated equine estrogens and medroxyprogesterone acetate. Methods: Three hundred fifty-seven postmenopausal women were selected randomly (in a blinded manner) to 12 months of treatment with 1 mg norethindrone acetate/5 microg ethinyl estradiol, placebo, or open-label 0.625 mg conjugated equine estrogens/2.5 mg medroxyprogesterone acetate (conjugated equine estrogens/medroxyprogesterone acetate [CEE/MPA]; ...
Metabolism of progesterone by placentas from several mammalian species in vitro. 20-alpha-Hydroxysteroid oxidoreductase (20-alpha-HSDH) activity and 20-alpha-dihydroprogesterone concentration (20-alpha-DHP) reach peak values in the human placenta after vaginal delivery. To determine if these findings are unique to the human, we measured 20-alpha-HSDH activity as well as endogenous progesterone (P) and 20-alpha-DHP concentration in the soluble supernatant fraction of placental tissues obtained from rodents (rat, rabbit, guinea pig), ungulates (horse, zebra, giraffe, cow), and primates (squirrel monkey, orangutan, man). P concentration was very low in rodents (mean 0.60 ng/m...
