Acute lameness associated with osseous metastasis of a peri-renal carcinoma in a horse.
Abstract: We present a case of aggressive metastatic carcinoma in a horse that was initially presented for shoulder lameness. Although radiography and scintigraphy were useful for localising a lesion in the proximal humerus, subsequent development of non-specific signs of systemic disease prompted further evaluation. Haematology and blood biochemistry, urinalysis and ultrasonography were all instrumental in identifying renal involvement. A diagnosis of a peri-renal mass causing secondary renal failure prompted euthanasia of the horse because of the poor prognosis. Antemortem findings were supported by necropsy, with secondary lesions also identified in the spleen, liver, 8th left rib and proximal humerus. Histological examination yielded a diagnosis of undifferentiated metastatic carcinoma.

This research article discusses a case of a horse diagnosed with acute lameness due to an aggressive metastatic carcinoma, focusing on diagnostic measures and findings, and the horse’s unfortunate prognosis.
Overview of the Case
- A horse was initially presented with shoulder lameness, prompting diagnostic procedures. Radiography and scintigraphy were primarily used to locate a lesion in the proximal humerus. The development of non-specific signs of systemic disease later required further evaluation.
Diagnostic Measures and Findings
- In response to these non-specific signs of systemic disease, various diagnostic measures were implemented. Haematology and blood biochemistry were helpful in identifying abnormalities in blood parameters. Urinalysis provided evidence of issues with kidney function. Ultrasonography was instrumental in identifying renal involvement in the disease pathology.
- These diagnostic measures collectively pointed towards a diagnosis of a peri-renal mass causing secondary renal failure. This suggested a significant link between the lameness in the horse’s shoulder and a deeper, systemic issue.
Prognosis and Final Measures
- Given the severity of its condition and the dismal prognosis, veterinarians chose to euthanise the horse. An autopsy confirmed the findings made while the horse was alive.
- In addition to the primary renal mass, secondary lesions were also found in various parts of the horse’s body, including the spleen, liver, 8th left rib, and proximal humerus. This revealed the metastatic nature of the carcinoma.
- Upon histological examination, the final diagnosis was determined to be an undifferentiated metastatic carcinoma. This implies that the cancer cells were immature or undifferentiated, indicating an aggressive form of cancer, hence the poor prognosis for the horse.
