Clinical application of prokinetics.
Abstract: Intestinal stasis or ileus is a significant cause of mortality and morbidity in horses and has been attributed to a variety of causes, including loss of intrinsic or extrinsic electrical activity, incoordination of contractile activity from regional stimuli, and dissociation between electrical and mechanical activity. Proposed mechanisms include systemic shock, electrolyte disturbances, persistent luminal distention, ischemia, inflammation, peritonitis, endotoxemia, and anesthesia. Because the cause of ileus is likely multifactorial, a variety of pharmaceutics have been used to target specific causes. Prokinetics are defined as agents that facilitate or enhance the net movement of feed material down the length of the intestinal tract and do not simply produce an uncoordinated increase in local contractile activity. The primary objective of pharmaceutic intervention is to augment the pathways that stimulate motility or attenuate the inhibitory neurons that predominantly suppress activity. The objective of this article is to summarize the actions of prokinetic agents available and suggest clinical applications.

This research paper investigates the use of prokinetic medications to enhance intestinal motility in horses suffering from ileus, a condition characterized by the stagnation of feed material in the intestinal tract.
Concept of Ileus in Horses
- The paper begins by defining ileus, a condition also known as intestinal stasis, which is a widespread cause of death and disease in horses.
- This condition refers to the impairment of the intestinal movement of feed material, which can be caused by a variety of factors such as a loss in the electrical activity responsible for contractile activity in the intestine, or a lack of coordination between regional stimuli and this contractile activity.
- Other factors leading to ileus may include systemic shock, electrolyte disturbances, persistent luminal distention, ischemia, inflammation, peritonitis, endotoxemia, and anesthesia.
Role of Prokinetics in Treating Ileus
- Given the multifaceted nature of the causes of ileus, the research paper discusses the use of different pharmaceutics to target specific causes, with a specific focus on prokinetics.
- Prokinetics are medications that facilitate or enhance the movement of feed material down the intestinal tract, without simply increasing uncoordinated local contractile activity.
- The primary aim of these drugs is to enhance the pathways that stimulate intestinal motility, or reduce the impact of inhibitory neurons that predominantly suppress this activity.
Aim of the Research Paper
- The paper aims to cover the actions of various prokinetic agents and suggest potential clinical applications for these drugs in the treatment of ileus in horses.
- By reviewing the available prokinetic agents and their clinical uses, the paper aims to assist in the advancement of treatment avenues for ileus, and thereby improve outcomes for horses afflicted by this condition.
