Giant right atrial diverticulum in a foal.
Abstract: A 5-month-old male Thoroughbred foal with a history of chronic septic arthritis of the tibiotarsal joint and recent respiratory distress was euthanized and a postmortem examination performed. A giant diverticulum communicating with the lateral aspect of the right atrial cavity of the heart was observed. Histologically, the wall was comprised of myocardial tissue containing cavernous vascular spaces. There was gross and histologic evidence of right-sided heart failure. Congenital right atrial diverticula are rare anomalies in humans and have not previously been reported in foals.

The research paper talks about a rare case of a 5-month-old Thoroughbred foal with a large, unusual pouch (diverticulum) found on the right side of its heart, which was discovered during a postmortem examination. The animal had a history of chronic septic arthritis and recent respiratory distress, and showed evidence of heart failure. Such a congenital anomaly is seldom seen in humans and is reported for the first time in foals.
Identification of the Problem
- The subject of the research is a 5-month old male Thoroughbred foal that had a history of chronic septic arthritis of the tibiotarsal joint and recent episodes of respiratory distress.
- The foal was euthanized, and a postmortem examination was performed to understand the cause behind its distress.
Key Findings
- Upon examination, researchers found a giant diverticulum – a hollow protruding pouch that forms in the wall of a vessel or an organ.
- This pouch was communicating with the lateral aspect of the right atrial cavity of the foal’s heart. This meant that there was an anomaly in the heart, which is a vital organ.
- Upon studying the tissues of the wall of this diverticulum (histology), it was found that the wall was comprised of myocardial tissue (heart muscle tissue) which contained cavernous vascular spaces (large blood vessels).
Evidence of Heart Failure
- Both gross and histologic (tissue-related) evidence indicated right-sided heart failure. This explains the respiratory distress seen in the foal, as heart failure often causes fluid to back up in the lungs, resulting in shortness of breath.
- This kind of malformation where a pouch forms in the right atrium of the heart (right atrial diverticulum) is very rare, even in humans.
- This is the first reported case of a right atrial diverticulum in a foal.
Conclusion
- This paper highlights the importance of thorough postmortem examinations in identifying rare or previously unreported anomalies in animal health.
- While the results of this case are inherently tragic, understanding the causes can lead to better diagnostic measures and interventions in animals displaying similar symptoms in the future.
