Gluteal pyomyositis associated with septic proximal femoral physitis in a 12-day-old Standardbred filly foal.
Abstract: A 12-day-old Standardbred filly foal was presented with an acute onset hindlimb lameness of 24 hours duration. Initial ultrasonographic evaluation of the right gluteal region revealed oedematous change to the muscle architecture. Conservative therapy consisting of antimicrobials and stall rest was initiated. Forty-eight hours after admission a localised gluteal pyomyositis had developed. This was drained twice by percutaneous aspiration. Four days after admission the foal was euthanased. Necropsy examination revealed a septic proximal femoral physis with no grossly apparent joint involvement.

This study outlines the case of a 12-day-old filly foal diagnosed with gluteal pyomyositis and septic proximal femoral physitis, which led to euthanization after conservative treatments failed.
Introduction and Background
- This research focuses on a case study involving a young 12-day-old Standardbred filly foal.
- The foal was initially presented with an acute onset hindlimb lameness that had persisted for 24 hours.
Diagnosis
- An initial ultrasound examination of the affected area, the right gluteal region, showed changes in the muscle architecture, indicating inflammation.
- Two days after the start of treatment, the same area developed a condition identified as gluteal pyomyositis, a bacterial infection within the muscle.
- Simultaneously, a septic proximal femoral physis was discovered, a condition in which the growth plate near the top of the femur becomes infected.
Treatment and Outcome
- A conservative approach was taken in treating the foal’s conditions.
- This involved using antimicrobials to fight the infection and keeping the foal rested in its stall.
- Despite these efforts, the infection worsened and the affected area had to be drained twice by percutaneous aspiration, a procedure to remove fluid using a needle.
- Unfortunately, the foal’s condition did not improve, leading to the decision to euthanize it four days after it was brought in for treatment.
Post Mortem Examination
- Following euthanasia, a comprehensive necropsy examination was performed.
- This investigation revealed that the infection had spread to the proximal femoral physis, causing septicemia.
- Interestingly, there was no apparent joint involvement, leading researchers to believe that the infection was localized to the muscle and proximal femoral physis.
