[How to manage a rectal lesion in horses in clinical practice].
Abstract: The most feared complication after a rectal examination of horses and ponies is to cause a tear of all the layers in the intraperitoneal part of the rectum. The description of the anatomy is an attempt to explain that the rectal tear is mostly located in the dorsal or dorsolateral part of the rectum. This article describes a relatively simple suture technique for the rectal tear by using the needle of Deschamp which is modified by Professor B. Huskamp. Furthermore there is a description of this method as treatment of two horses in practice circumstances. The issue whether causing a rectal tear should be regarded as a case of malpractice is discussed. Advises as prevention of rectal tears are given.

The research article deals with the management and repair of rectal tears in horses, focusing on the anatomy and the localization of such lesions. It provides a detailed description of a simple suture technique for treating rectal tears, uses specific case studies for illustration, and also addresses the question of whether causing a rectal tear should be considered malpractice.
Anatomy and Localization of Rectal Tears
- This research starts by laying a foundation in regards to the anatomy of horses, and where rectal tears are most commonly located – the dorsal or dorsolateral part of the rectum.
- Knowledge of the specific anatomy is essential to understanding why rectal tears occur in these areas and help inform appropriate treatment and repair techniques.
Suturing Technique for Rectal Tears
- The study introduces a suture technique, modified by Professor B. Huskamp, for rectal tears employing the Deschamp needle. This particular technique is cited as being relatively simple to execute.
- This points to the possibility of the method being easily adoptable in regular veterinary practices, potentially improving the outcomes for horses suffering from rectal tears.
Case Studies
- For practical demonstration, the research features two case studies where the proposed suture technique has been used to repair rectal tears.
- The inclusion of these case studies serves as practical evidence of the technique’s effectiveness, and also offers practitioners a realistic view of the procedure in action.
Rectal Tear as a Case of Malpractice
- This research goes beyond the technical and scientific aspects to delve into ethical considerations, posing the question of whether unintentional causing of a rectal tear could be seen as malpractice.
- This component brings to light the legal and professional implications in the case of rectal tears, which can prompt practitioners to consider their actions more deliberately and potentially avoid unnecessary injuries.
Prevention of Rectal Tears
- Finally, the study provides advice on the prevention of rectal tears, hence looking at rectal lesion management from a holistic, preventative angle rather than just treating existing injuries.
- This piece of advice can enable more comprehensive care and reduce the likelihood of horses suffering from rectal tears in the first place.
