Sepsis of the common digital extensor tendon sheath secondary to hemicircumferential periosteal transection in a foal.
Abstract: A 4-month-old Quarter Horse was admitted for evaluation of infection that developed following bilateral hemicircumferential periosteal transection and elevation. Sepsis of the common digital extensor tendon sheath was diagnosed. Treatment included lavage of the tendon sheath, placement of an indwelling drain for lavage after surgery, establishment of drainage, and administration of antimicrobial and anti-inflammatory drugs. Three months after treatment, the foal was clinically normal.

A 4-month-old Quarter Horse was successfully treated for an infection in its digital extensor tendon sheath, which was a complication of periosteal transection and elevation surgery.
Study Overview
The research article presents a case study of a 4-month-old Quarter Horse that developed an infection, specifically sepsis in the common digital extensor tendon sheath, following a surgery known as bilateral hemicircumferential periosteal transection and elevation.
Methodology and Intervention
- The treatment included several strategies. First was the lavage or surgical cleansing of the affected tendon sheath, which aims to remove infectious materials and debris to reduce the inflammatory response and promote healing.
- An indwelling drain was placed for continuous lavage after the surgery. This allows for efficient removal of the aforementioned infectious materials and debris over time.
- A means of drainage was also established to further aid in the removal of infectious agents or other undesirable materials.
- Finally, antimicrobial and anti-inflammatory drugs were administered to combat the infection and reduce inflammation and pain associated with the condition.
Results and Conclusion
- The outcome was extremely positive, with the foal returning to a ‘clinically normal’ status three months after treatment.
- This indicates a complete recovery based on clinical observations and suggests the successful eradication of the infection as well as the effective healing of the surgical site and surrounding tissues.
- The study highlights the efficacy of this combined approach in treating this kind of infection in horses, providing critical information for veterinary practice and possibly improving clinical outcomes for similar cases in the future.
