Streptococcal toxic shock in a horse.

Abstract: A 14-year-old horse was admitted to the veterinary hospital for treatment of tachycardia and lethargy. Initial diagnoses were ventricular tachycardia and renal dysfunction. During hospitalization other findings included fever, renal failure, hepatic failure, hypotension, and intermittent ventricular arrhythmias. Bacteriologic culture of 2 blood samples collected during febrile crises 7 days apart yielded Streptococcus mitis. These culture results along with other clinical and physical examination findings fulfill the criteria for a diagnosis of streptococcal toxic shock syndrome, previously described for humans and dogs. To our knowledge this is the first reported instance of this disease in a horse.

This research article presents a unique case of Streptococcal toxic shock syndrome detected in a horse, making it reportedly the first case ever of this disease in horses, as it is typically found in humans and dogs.

Introduction to the Case

  • The case in question involves a 14-year-old horse that was brought to a veterinary hospital due to lethargy and tachycardia, which is an abnormally rapid heart rate. The initial diagnosis of the horse included ventricular tachycardia and renal dysfunction.

Progression of the Disease

  • The horse’s condition deteriorated during the hospitalization period. It developed fever, renal and hepatic failure, indicating severe dysfunction of both the kidneys and the liver. Additionally, the horse was suffering from hypotension, which is low blood pressure, and had intermittent ventricular arrhythmias, implying irregular patterns in the ventricles’ rhythm.

Final Diagnosis

  • A bacteriologic culture of two blood samples collected a week apart during the horse’s febrile crises returned positive for Streptococcus mitis. Streptococcus mitis is a bacterium that is a part of the normal flora of the human mouth and is rarely pathogenic in humans. However, in immunocompromised hosts, it can sometimes cause infections.
  • This positive result for Streptococcus mitis in the blood culture, coupled with the clinical symptoms and physical examination findings, led to the conclusion that the horse had Streptococcal toxic shock syndrome.
  • This disease, as mentioned before, is usually witnessed in human and dog populations and is characterized by all the symptoms mentioned above.

Conclusion and Significance of the Case

  • This case marks the first recorded instance of Streptococcal toxic shock syndrome in a horse. This finding is significant as it adds a new dimension to veterinary medicine and introduces a fresh perspective on how diseases typically associated with humans and dogs might also impact other animals like horses.
