Use of continuous-flow peritoneal dialysis for the treatment of acute renal failure in an adult horse.
Abstract: A 15-year-old Paso Fino gelding was evaluated because of acute renal failure following an episode of exertional rhabdomyolysis. The horse was azotemic and treated conservatively at another referral practice with no improvement in the azotemia. With conservative treatment and intermittent peritoneal dialysis, the horse had minimal improvement. Continuous-flow peritoneal dialysis (CFPD) was instituted on day 7 and continued for 3 consecutive days. Dramatic changes in the horse's attitude and serum creatinine concentration were detected within the first 24 hours of CFPD treatment. The horse remained hospitalized for 23 days; within 3 months of discharge, serum BUN and creatinine concentrations had returned to within the reference ranges and the horse had resumed normal activity. In adult horses, it appears that CFPD can be used to successfully treat acute renal failure that is refractory to conventional treatments.

The research article discusses the application of Continuous-Flow Peritoneal Dialysis (CFPD) in the treatment of acute renal failure in an adult horse, and how it demonstrated positive results, including marked improvements in the health and attitude of the horse, following failure of initially conservative treatments.
Background of the Research
- The study was conducted on a 15-year-old Paso Fino gelding (type of horse), that had acute renal failure as a consequence of an episode of exertional rhabdomyolysis.
- Prior to the start of the research intervention, the horse had been treated using conservative methods at another referral practice. However, the condition of the horse, specifically azotemia, showed no improvement.
- The application of intermittent peritoneal dialysis as a treatment resulted in minimal improvements in the horse’s condition.
The Research Intervention and Results
- Given the lack of success with conservative treatments, the study proceeded with the application of Continuous-Flow Peritoneal Dialysis (CFPD) starting at day 7 and continuing for three consecutive days.
- Within the first 24 hours of the CFPD treatment, noticeable improvements were apparent both in the horse’s behavior and serum creatinine concentration.
- The horse stayed in the hospital for 23 days. Within three months following discharge, the serum BUN and creatinine concentrations returned to the normal levels (within reference ranges).
- Additionally, the horse was able to resume normal activity, suggesting a recovery from the initial acute renal failure.
Conclusion deriving from the Research
- Based on the effective treatment and recovery of the horse following the CFPD intervention, the research suggests that CFPD could serve as a successful treatment method for cases of acute renal failure in adult horses.
- This approach might be especially applicable in cases where the kidney failure is unresponsive to conventional treatments.
