Development of a composite orthopaedic pain scale in horses.

Abstract: This study addresses development and validation of a composite multifactorial pain scale (CPS) in an experimental equine model of acute orthopaedic pain. Eighteen horses were allocated to control (sedation with/without epidural analgesia - mixture of morphine, ropivacaine, detomidine and ketamine) and experimental groups: amphotericin-B injection in the tarsocrural joint induced pain and analgesia was either i.v. phenylbutazone administered post-induction of synovitis, or pre-emptive epidural mixture, or a pre-emptive combination of the 2. Inter- and intra-observer reproducibility was good (0.8 Publication Date: 2007-12-03 PubMed ID: 18061637DOI: 10.1016/j.rvsc.2007.10.011Google Scholar: Lookup

Summary

This research summary has been generated with artificial intelligence and may contain errors and omissions. Refer to the original study to confirm details provided. Submit correction.

The research paper focuses on devising an effective and efficient composite pain scale (CPS) to understand and manage the orthopaedic pain experienced by horses. In the study, different behavioural and physiological criteria were analyzed to measure the intensity of equine orthopaedic pain and various pain management protocols are being tested.

Study Design and Approach

  • The study signifies an experimental model involving 18 horses.
  • The horses were divided into control and experimental groups.
  • In the control group, the horses were sedated with or without the use of epidural analgesia comprising a mixture of different drugs: morphine, ropivacaine, detomidine, and ketamine.
  • An induced pain treatment was administered to the experimental group by injecting amphotericin-B into the horse’s tarsocrural joint.
  • Pain in experimented horses was alleviated utilizing intravenous administration of phenylbutazone post pain induction, or by using a preemptive epidural mixture, or a combination of both.

Observations and Results

  • The research highlights a good level of inter- and intra-observer reproducibility in their observations.
  • Out of all the behavioural indices used to measure pain, the reaction to palpation of the painful area and the posture of the horse were found to be most significant.
  • A lesser value was attached to pawing on the floor, kicking at the abdomen, and head movements.
  • The composite pain scale showed strong statistical correlation with non-invasive blood pressure and blood cortisol levels.

Conclusion and Implication

  • The research reinforces the potential value of specific behavioural and physiological criteria in determining equine orthopaedic pain intensity.
  • It demonstrates that pre-emptive, multimodal analgesia offers better management of pain as compared to the two other methods tested.
  • This suggests the efficacy of CPS in not just understanding but also managing equine orthopaedic pain, serving as a significant step in veterinary research and practice.
