Use of Antimicrobials in a French Veterinary Teaching Hospital: A Retrospective Study.

Abstract: Antibiotic resistance has become a major concern for not only human health, but also for animal health. To preserve the efficacy of antibiotics, it has become essential to establish measures to regulate the prescription of antibiotics to ensure their prudent use. In France, these measures have been translated into regulations for animal health since 2015, with the publication of three important regulatory texts. The results obtained on a national scale in terms of reducing the use of antibiotics have been satisfactory. The aim of our study was to evaluate the differences related to the prescription of antimicrobials at the veterinary teaching hospital of the Veterinary School of Lyon (CHUV) before and after the implementation of French regulations. Prescriptions and consumption of antimicrobials were examined, along with bacteriological analyses, for the period of 2014-2020, for companion animals and horses. The most frequently prescribed compounds were broad-spectrum antimicrobials, including penicillins with β-lactamase inhibitors, as well as first-generation cephalosporins tetracyclines and sulfamides. The prescription and consumption of critically important antibiotics (CIA) strongly decreased during the study period, with an increase of bacteriological analyses. This study shows the interest of having computerized tools to monitor the use of antimicrobials to implement corrective measures if needed.

This research evaluated how the use of antimicrobials changed at a veterinary teaching hospital in Lyon, France after regulations were introduced. Results showed that the use of crucially important antibiotics significantly decreased, with an increase in bacteriological analysis, demonstrating the value of technology in monitoring antimicrobial use.

Objective

The study aimed to assess changes in antimicrobial prescriptions at the Veterinary School of Lyon’s teaching hospital before and after the implementation of French regulations designed to govern antibiotic use.

Background

  • Antibiotic resistance has become a major concern for human and animal health alike, resulting in the need for measures to regulate antibiotic prescriptions and ensure their prudent use.
  • In response to this, France introduced regulations governing animal health in 2015, resulting in three key regulatory texts.
  • The efforts to reduce antibiotic use at a national level has been satisfactory.

Methodology

  • The study examined prescriptions and consumption of antimicrobials, along with bacteriological analyses, covering the period 2014-2020, for companion animals and horses.
  • The use of antibiotics of critical importance (CIA) was a particular focus.

Key Findings

  • The most commonly prescribed antimicrobials were broad-spectrum ones, including penicillins with β-lactamase inhibitors, as well as first-generation cephalosporins tetracyclines and sulfonamides.
  • There was a strong decrease in the prescription and consumption of critically important antibiotics over the course of the study period.
  • There was an increase in bacteriological analyses during the study period.

Implications

  • The study highlights the efficacy of regulatory measures in reducing the use of critically important antibiotics in veterinary medicine.
  • It also demonstrates the valuable role that computerized tools can play in monitoring the use of antimicrobials, allowing for timely implementation of corrective measures where needed.
