Laryngeal hemiplegia in draft horses. A review of 27 cases. Case records of 27 draft horses with laryngeal hemiplegia were reviewed. Twenty-one horses were treated by ventriculectomy with or without prosthetic laryngoplasty, and 17 owners were contacted to determine the results. Fifteen horses improved after surgery and were able to perform to the owners' expectations. Performance improved significantly and hospitalization was shorter after ventriculectomy alone. Vascular injury associated with naturally occurring strangulating obstructions of the equine large colon. Ten horses were euthanatized before, during, or after surgery to correct severe volvulus of the large colon. At surgery, the colonic serosa changed from blue-gray, blue or purple toward a more normal pink in seven horses after the volvulus was corrected. The mucosa consistently remained black or dark red. Equine post-anesthetic lameness. A retrospective study. The incidence of post-anesthetic lameness in 655 horses undergoing 733 anesthetic episodes over a 3 year period was 6.4%. Nineteen factors previously reported or proposed to play a role in the development of post-anesthetic lameness were evaluated statistically. Only hypotension and the duration of the anesthetic period were significant factors.
Arytenoidectomy for advanced unilateral chondropathy with accompanying lesions. A noncurrent prospective study of nonworking horses with arytenoid chondropathy was conducted to confirm a clinical impression that horses with unilateral chondropathy and accompanying lesions had a poorer prognosis after arytenoidectomy than horses with bilateral or uncomplicated unilateral chondropathy. Transendoscopic Nd:YAG laser surgery for treatment of epiglottal entrapment and dorsal displacement of the soft palate in the horse. Transendoscopic neodymium:yttrium-aluminum-garnet (Nd:YAG) laser was used to treat 12 standing horses with epiglottic entrapment (EE) or dorsal displacement of the soft palate (DDSP), or both. In four horses, transendoscopic laser staphylectomy was performed. The most common presenting complaints were respiratory stridor, cough, and exercise intolerance. Vascular response of the equine radius to cerclage devices. The arterial blood supply to the diaphysis of the radius, and the vascular reaction when three cerclage devices (cerclage wire, Parham-Martin band, and a partially contacting band) were applied 2 cm apart, were studied in six adult ponies. The opposite radial diaphysis served as the control. Ponies were euthanatized at weeks 4 and 8. A centrifugal blood flow pattern was demonstrated by microangiography. Larger numbers of medullary arteries exist in equine than in canine and human long bones. Changes in centrifugal blood supply were not identified beneath any of the cerclage devices.
Endoscopy of the digital flexor tendon sheath in horses. An arthroscopic procedure for examination of the digital flexor tendons and tendon sheath was developed in 16 equine limbs and 12 horses. Distension of the tendon sheath and insertion of the arthroscope was accomplished through a cul-de-sac on the palmar or plantar surface of the tendon sheath 1 to 2 cm palmar or plantar to the digital neurovascular structures and between the annular ligament and proximal digital annular ligament. A comparison of repair methods for gap healing in equine flexor tendon. In nine horses (18 forelimbs), a 3 cm section of superficial digital flexor tendon was removed and the tendons were repaired with immobilization for 6 weeks and (1) no suture (n = 6); (2) a double locking loop tenorrhaphy with carbon fiber (n = 6); or (3) a double locking loop tenorrhaphy with size 2 nylon suture (n = 6). Evaluation of 25%, 50%, and 67% nitrous oxide with halothane-oxygen for general anesthesia in horses. Twenty-five percent, 50%, and 67% nitrous oxide was administered to 12 horses anesthetized with halothane and oxygen. Compared to halothane-oxygen alone, there was no significant difference in heart rate, systolic, diastolic, or mean blood pressure values, arterial pH, PaCO2, or plasma bicarbonate values when nitrous oxide was included. Treatment of advanced squamous cell carcinomas involving the equine cornea. Corneal squamous cell carcinomas with surface areas of more than 2.0 cm were diagnosed in 26 eyes of 25 horses. The depth of the lesion into the corneal stroma and involvement of the ocular adnexa were important determinants for surgical treatment. Corneal tumors that extended into adjacent palpebral conjunctiva, eyelid, or orbit were not amenable to complete excision, while those that involved only cornea and bulbar conjunctiva were treated surgically by keratectomy and beta radiation. A surgical approach to the ramus of the mandible in cattle and horses. Case reports of a bull and a horse. A surgical approach to the ramus of the mandible was developed. Subperiosteal elevation of the masseter muscle from the ramus allowed reduction and plate fixation of ramus fractures in a bull and a horse. Results of follow-up examinations, 2 years after surgery, showed excellent functional and cosmetic results.
Proximal interphalangeal arthrodesis in the horse. A retrospective study and a modified screw technique. Arthrodesis of the proximal interphalangeal joint was used to treat lameness resulting from osteoarthrosis and for closed subluxation in 35 horses. Six horses had bilateral arthrodeses. Several conventional internal fixation techniques and a three converging screw method were used. Criteria for success included the horse performing its previous or intended athletic activity and the owner being satisfied with the outcome. Epiglottic augmentation in the horse. Epiglottic augmentation with injectable bovine collagen or an autogenous or allogenous auricular cartilage graft was performed in 12 horses with endoscopically and radiographically normal epiglottises. The grafting procedures were easy to perform and did not cause apparent discomfort. Cartilage graft extrusion or resorption may have occurred, but was not seen by endoscopy and lateral laryngeal radiography. Pulse oximetry in horses. The clinical usefulness of two pulse oximeters was evaluated at two probe sites in nine anesthetized horses. The hemoglobin saturation determined by the pulse oximeters (SaOx) was compared with the hemoglobin saturation calculated from the measured arterial oxygen tension (SaO2). The mean and standard deviation (SD) were calculated from the differences in saturation measurements, over the saturation range of 80% to 100%, for each oximeter used at the tongue probe site and for one oximeter used at the ear. A 2 month old quarter horse colt had multiple skeletal abnormalities of the left fore limb, including two supernumerary metacarpal bones, two supernumerary carpal bones, abnormal carpal bone development, and a valgus deformity originating at the carpal joint. The largest supernumerary metacarpal bone was removed completely, a portion of the smaller supernumerary metacarpal bone was removed, and the leg was placed in a tube cast. Surgical repair of tibial tuberosity avulsion fractures in four horses. Four horses, 2 to 17 years old, were treated for unilateral avulsion fractures of the tibial tuberosity. Two horses were treated successfully with tension band wiring or plating in combination with lag screw fixation. One horse was euthanatized because of implant failure during recovery and one was euthanatized on day 11 because a longitudinal fracture of the tibial tuberosity occurred through the plane of the screws used for stabilization.
Upper airway function during maximal exercise in horses with obstructive upper airway lesions. Effect of surgical treatment. Upper airway pressure was measured during maximal exercise in 10 Thoroughbred racehorses with naturally occurring upper airway obstruction. Left laryngeal hemiplegia and arytenoid chondropathy resulted in substantial increases (30-40 cm H2O) in inspiratory upper airway pressure (Pl), whereas complicated aryepiglottic entrapment and subepiglottic cysts produced only modest increases (15 cm H2O) in Pl. Evaluation of an interfragmentary compression system for the repair of equine femoral capital physeal fractures. Femoral neck and proximal epiphyseal lengths were measured in 37 femurs from 19 cadaver foals that were 1 day to 12 months old to determine the applicability of a human interfragmentary compression system to equine femoral capital physeal fractures. Because components of the implant system are available only in fixed sizes, its use was possible in foals older than 5 weeks of age, but not in younger foals. Effects of left recurrent laryngeal neurectomy, prosthetic laryngoplasty, and subtotal arytenoidectomy on upper airway pressure during maximal exertion. Upper airway pressure was measured with a nasotracheal catheter system and a portable pressure transducer in 10 normal horses during maximal exercise before and after left recurrent laryngeal neurectomy. Measurements were repeated 16 weeks after prosthetic laryngoplasty (5 horses) or subtotal arytenoidectomy (5 horses). Osteostixis for incomplete cortical fracture of the third metacarpal bone. Results in 11 horses. Eleven horses with acute or chronic incomplete cortical fractures of the left or right third metacarpal bone (McIII) were treated with surgical puncture (osteostixis). The fractures were diagnosed by physical examination and radiography. Four to eight holes, 2.7 or 3.5 mm in diameter, were drilled in the fractured bone. Postanesthetic equine myopathy suggestive of malignant hyperthermia. A case report. Signs of malignant hyperthermia, including progressive increases in PaCO2, skin temperature and heart rate, and elevated serum levels of potassium, inorganic phosphate, and creatine kinase, were identified in a halothane-anesthetized horse. Treatment was discontinuing halothane administration, applying ice and cold fluids, and hyperventilating with 100% oxygen. Retrospective evaluation of repeat celiotomy in 53 horses with acute gastrointestinal disease. Fifty-three of 648 horses (8.2%) treated surgically for acute gastrointestinal obstruction were subjected to repeat celiotomy. Forty horses (75%) recovered from anesthesia after repeat celiotomy, and 26 horses (49%) were discharged from the clinic. Excluding two horses lost to follow-up, 10 of 51 horses (20%) survived long term.
