Fregin GF.The approach to pre-purchase evaluation of the equine cardiovascular system is discussed with emphasis on cardiac auscultation and on the terminology that should be used when reporting clinical findings. Innocent cardiac murmurs and arrhythmias are contrasted with those that should be considered as reliable signs of cardiovascular disease. Ultrasonography and electrocardiography are recommended to establish baseline values for future reference, to support opinions rendered by the examining veterinarians, or to provide for diagnosis and treatment.

Ball MA, Weldon AD.A 5-year-old Appaloosa gelding was presented with a history of intermittent multiple joint swelling, weight loss, and anemia. Physical examination and clinical pathology revealed a grade IV/VI holodiastolic murmur, louder on the left, and a marked hyperproteinemia. Echocardiography of the heart demonstrated a large vegetative lesion on one of the aortic valve cusps. Blood cultures did not elucidate the causative organism, and the disease was refractory to empiric antibiotic therapy. The horse was euthanized after approximately 2 months of therapy.

Thoracic radiographs, cardiac ultrasound, cardiac catheterization, and differential blood gases were utilized to tentatively diagnose tetralogy of Fallot. Postmortem examination confirmed tetralogy of Fallot and additionally revealed segmental uterine aplasia.

Echocardiographic findings (diastolic fluttering of the septal leaflet of the mitral valve, left ventricular volume overload, and incomplete aortic valve closure) were diagnostic for aortic valve insufficiency. Moderately thick fibrotic aortic valve leaflets were found at necropsy. Fenestrations were found in the aortic and pulmonic valve cusps. Congenital valvular disease may have led to aortic valvular insufficiency in this horse.

Reef VB.Cardiac disease in the equine neonate occurs infrequently. Murmurs are often heard in foals and are not considered significant unless they persist beyond 4 days of age. Congenital cardiac defects are the most common form of primary cardiac disease in the foal, with ventricular septal defects occurring most frequently. Other neonatal foal diseases such as ruptured bladders, white muscle disease, neonatal respiratory distress syndrome, and septicemia have secondary cardiac involvement.

In the normal motion of the mitral valve there is anterior motion noted during atrial systole. In 3 horses with atrial fibrillation the mitral valve motion had no anterior motion to correspond with atrial contractions. Quinidine returned the electrocardiogram of one horse to a sinus rhythm after which echocardiography demonstrated the reappearance of the anterior motion of the mitral valve.

Brown CM, Holmes JR.Using microtransducers, the intracardiac phonocardiogram was recorded in 8 horses from the right heart and in 6 of them also on the left side. All 4 heart sounds were recorded but not in all sites. Their timing was related to pressure events. Systolic ejection murmurs were recorded in both the pulmonary artery and the aorta. Diastolic murmurs were recorded in 2 horses with aortic regurgitation, with maximum amplitude in the left ventricle. The findings were similar to those reported in man and agree with some of the recent concepts on heart sound production.

Peritoneal fluid and blood samples both demonstrated an extreme leucocytosis and neutrophilia. At post-mortem both a cranial mesenteric arteritis and the congenital cardiac anomaly, Tetralogy of Fallot were found. The detailed clinical and post-mortem findings contributing to the case are described.
