This article reviews the evidence relating to its incidence, clinical findings, radiological observations, histopathology and certain aspects of respiratory physiology. It is proposed that EIPH is primarily caused by mechanical stress in the dorsocaudal region of the lung.

Endorphin systems are probably involved in the effectiveness of the twitch, since its action is blocked by naloxone and its application increases plasma concentrations of immunoreactive beta-endorphin. The mode of action of the twitch cannot be explained by the generally accepted theory of divertive pain and may resemble that of classical acupuncture.

Plasma cortisol levels were significantly (P less than 0.01) increased within 60 mins of injection of ACTH1-24 and immediately after exercise. The mean neutrophil/lymphocyte ratio altered significantly (P less than 0.01) at 240 mins after ACTH1-24 administration and at 180 mins after a training gallop. None of 6 stressed male horses had low cortisol levels but testosterone levels (0.81 +/- 0.15 nmol/l) were significantly lower than in healthy horses (1.86 +/- 0.31 nmol/l). The conclusion was made that adrenocortical insufficiency is not a common association of stress in race horses. The reduction in testosterone levels is probably a nonspecific response similar to that seen in other species.

Since ACTH induces the release of cortisol from the adrenal glands, blood cortisol level should be enhanced by electroacupuncture.
